Perforator surgery

To avoid making cuts through damaged and weakened skin, a minimally invasive technique was described a few years ago. This is SEPS (subfascial endoscopic perforator surgery). A tight tourniquet is applied to the thigh to allow the operation to proceed without much bleeding. The cut is made at the upper calf and a camera advanced down the leg under the fascia (the tough tissue that surrounds the muscles and through which the perforator veins pass).

Through the camera the perforator can be seen and special clips applied before it is divided.

The wound is closed and a bandage applied as for long saphenous surgery.
